Your Doctor's'
Prescription is needed for your glasses and contact
lens orders to be filled. We honor any doctors remakes at no charge
to the patient. A copy of your records are kept on file and confidential.
Lisa VonAspern, ABOC follows all HIPPA privacy laws. The finest
care for your prescription satisfaction is all that matters. All
spectacle lens fabrication is performed by our lab, Metro Optics. |

Warranties and
Policy's Nonadaptation
to progressive (no-line bifocals) is 30 days. Your doctor's change of
prescription is honored within 30 days as well. Frame warranties are
through the manufacturer for defects up to one year. Lenses with our
premium non-glare Process. (anti-reflective). is a 2 year warranty!!
A case, lens cleaning cloth and spray cleaner comes with all glasses'
orders. All non-prescription sun wear purchases are final. |

| INSURANCE VSP
Patients have "out of network benefits" with For Your Eye's
Only, LLC. Most Insurance plans such as Coventry, Humana and LA BC BS
are Discount plans from 15% to 20% off their eyewear. We will be glad
to fill out all insurance forms for you to receive your benefits. Feel
free to call if you have any questions about your insurance. Also accepting
Medicaid and Kid Med for children's glasses up to age 21 |
